Has anyone had any experience in setting up some sort of plan for such a contingency?

In Canada we can do that with power of attorney's (POA) and the executor instructions from our will.

I have both POA's for my wife and my nephew has both POA's for me. If I become incapacitated he can run things for me which includes my responsibilities for my own POA of my wife.

If I die, he executes the direction that all bills are paid in the care of my wife until she dies and then the remaining instructions are executed.

    andres,

This blog from 2008 addresses a lot of your question - http://www.thealzheimerspouse.com/diebeforespouse.htm

After Lloyd's seizure, death and resuscitation in 2011, I went to our lawyer and had his will revised, set up POA's and DNR's and made out my will. My will states that he is to remain with my daughter Maria. In 2010, Maria's husband added a third floor to our house and their family moved back home with us. Maria volunteered to do this and rented her house so I could quit work and stay home with Lloyd. Everything goes to her and she will distribute personal effects as she sees fit. Her brother and sister understand the fairness of this. His children are not even aware of this as they are not aware of much else by their choices. I cannot begin to tell you how much peace it has given me to have all this in order. After the bad experience at the hospital in 2011 and no DNR, I swore it would never happen again. There will be no interference again when it is his time to go. He never recovered and he would have been at peace now if I had that DNR. Please, friends, get this all in order before you need it!
