    We've been given a form by the home health agency called "Patient Acknowledgements and Consents". I have never seen this before although we have used HH for about a year.

    There is a freedom of choice section that basically says "I choose this agency and I understand no other agency can provide HH to me while I am under their care."

    I don't totally trust this agency because they have tried to send therapists that were not ordered by the doctor or requested by us.

    Do I have to sign this? What if I don't? Or is this just standard paperwork that all the HH agencies use?

    I think this is just standard paperwork. I am now switching my dh from day care to home health...M

    When I had HH for my Mom, everything had to go through their agency except her prescriptions. She had a colostomy and I had to order
    her supplies through them which was difficult because they couldn't seem to get the kind she used. Thankfully, I had extras on hand.

    flouncy you can use any MC certified agency you choose-but only one or it is considered a duplication of service. Some agencies are affiliated with a medical supply service and that gets dicey. You should be able to choose your source of supplies. The agency will faciliate ordering supplies if it is their own company=but there again the final chloice is yours. Where it gets even more complicated is when certain drugs or supplies will be used for treatment. If a nebulizer and meds are ordered from the same pharmacy/DME company then medicare will pay for the meds, but not if they come from two different places. Did I confuse you even more?

    Each state may be different but the lady in charge of my DH's HH told me that I could, if needed, use one agencey for Personal care and another for Respit. Medicare pays for the Personal care and Medicade pays for respit.
